Safeguard Health Insurance (“we”) acts as an independent insurance producer. In helping you compare and enroll in health coverage, we may receive protected health information (PHI) such as your name, date of birth, household details, prescriptions, and existing coverage. We protect this information as required by the Health Insurance Portability and Accountability Act (HIPAA), the Gramm-Leach-Bliley Act, and applicable state law.
How we may use and disclose your information
- Quoting and enrollment — to obtain quotes and submit applications to insurance carriers and the Health Insurance Marketplace at your request.
- Service — to answer your questions, assist with claims, billing, and renewals, and provide consulting or membership services you purchase.
- Legal requirements — when required by law, regulators, or court order, or to prevent fraud.
- Business associates — with vendors (such as form, email, scheduling, and payment processors) bound by contract to protect your information.
We do not sell your health information, and we do not use it for marketing unrelated to the insurance products you asked about without your written authorization. Any use not described here requires your written authorization, which you may revoke at any time.
Your rights
- Inspect and obtain a copy of the information we hold about you.
- Request corrections to inaccurate or incomplete information.
- Request an accounting of disclosures we have made.
- Request restrictions on certain uses or disclosures and confidential communications.
- Receive a paper copy of this notice on request.
- File a complaint with us or with the U.S. Department of Health & Human Services Office for Civil Rights (hhs.gov/ocr) without retaliation.
