01

How Claims Work

A claim is a request for payment sent to your health plan for care you received. Most of the time, your provider submits it for you.

02

Step 1: Read the Denial Carefully

When a claim is denied or only partly paid, you'll usually receive an EOB and, for many types of coverage, a written denial notice (sometimes called an adverse benefit determination).

03

Step 2: Check for Simple Fixes

Before starting a formal appeal, see whether the problem can be fixed by correcting information.

04

Step 3: Understand Your Appeal Rights

If the denial isn't a simple error, you may appeal. Your rights depend on your type of coverage.

05

Step 4: Build Your Appeal

A strong appeal addresses the specific reason for denial with clear, organized evidence.

06

Step 5: Write and Submit the Appeal

07

Step 6: After the Decision
